Results from a real-world, European Spondyloarthritis (EuroSpA) Research Collaboration registry-based observational study investigating predictors of secukinumab effectiveness in 2,790 patients with psoriatic arthritis (PsA) were published in The Journal of Rheumatology by Heberg et al. Outcomes included overall and sex-specific predictors of Disease Activity Index for Psoriatic Arthritis based on 28 joints (DAPSA28) low disease activity (LDA) and DAPSA28 moderate response at 6 months, as well as secukinumab discontinuation at 12 months.
Key data: At 6 months, positive predictors of DAPSA28 LDA and DAPSA28 moderate response included male sex, C-reactive protein (CRP) >10 mg/L, fewer prior biologic/targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs), and a lower Health Assessment Questionnaire (HAQ) score. The absence of psoriasis (PsO), a higher fatigue score, and a higher tender joint count of 28 joints (TJC28) were predictors of secukinumab discontinuation within 12 months. Sex-stratified analyses identified lower patient pain score, lower HAQ score, and lower TJC28 as predictors of DAPSA28 LDA at 6 months in both sexes, while fewer prior b/tsDMARDs and CRP >10 mg/L were male-specific predictors.
Key learning: The findings provide insight into the predictors of DAPSA28 LDA and DAPSA28 moderate response to secukinumab, supporting better personalized treat-to-target strategies.
